Terms and Conditions for purchasing MedicAlert Foundation Gift Certificates:
- The gift certificate is non-transferable and not redeemable for cash
- The gift certificate is valid for 12 months only from the date of issue
- The value of the gift certificate must be redeemed as one transaction
- The gift certificate can be used towards the purchase of any product
- The gift certificate can be used as full payment of annual membership fee but not as part-payment
- The gift certificate is only redeemable by posting it to MedicAlert Foundation with a membership form/annual fee notice.
