You can return your completed application form with your payment in any of the following ways:

Application forms are also available by calling us on 1800 88 22 22 (Monday-Friday, 9am - 5pm CST) or from your local doctor's surgery, pharmacy or hospital.
A membership pack (including your individually engraved emblem) will be posted to you. Please allow approximately 10 working days for delivery if no adjustments required.
Over the phone
You can also order over the phone by calling us on 1800 88 22 22 (Monday-Friday, 9am - 5pm CST) and speaking with one of our friendly Membership Services team members. Please have your information and credit card details ready.
